Switching to better care is easier than you think.

If you already receive services through a state waiver program like SOURCE, CCSP, or COMP/NOW, follow these steps to switch your provider to Tru Home Care:

Step-by-Step Instructions:

Switching to Tru Home Care is simple:

  1. Call Your Case Manager or Service Coordinator

  2. Ask to speak with your current coordinator.

  3. Request a Provider Change

  4. Let them know you’d like to switch your provider to Tru Home Care.

  5. Select Option 4 (if applicable)

  6. On the automated line, choose Option 4 to speak to someone about provider changes.

  7. Provide Our Company Info

  8. Give them our name and contact details:

    • Tru Home Care

    • Phone: (229) 668-0183
